We’ve got you covered with options that empower you to choose benefits right for you! You have 30 days from your hire date to enroll.
Full-time, active, non-union employees working at least 30 hours per week are eligible to participate in Azuria’s benefits plans.
- Salaried and hourly office employees are eligible on date of hire.
- Hourly field employees are eligible after 90 days of employment.
Changing Benefits Outside Enrollment
You may make changes to your enrollment within 30 days from a qualified life event, such as marriage, birth, adoption or divorce. Watch this video to learn more.
Working Spouse Provision
Your eligible dependents and spouse can join your medical plan provided through Azuria. However, spouses with access to their own employer’s medical coverage cannot enroll in Azuria’s Medical Plan. They are eligible for vision and dental coverage.
